Youth Registration Form

Please click on the "Register" button to fill out the registration form for all your youth. This form must be filled out by Parent/Guardian Church Center account. The form filled out by child Church Center account is invalid.

For youth who join both Sunday Service and small group, please select "FLINT Middle School Group & Sunday Service" or "LIFELINE High School Group & Sunday Service" based on the grade.

For youth who join Sunday Service without joining small group, please select "Sunday Service Only".

The form you submitted is valid until your student graduates from high school/Lifeline.

若以「孩子帳號」填寫此表單，則表單無效。

同時參加主日崇拜及小組的青少年，請依年級選擇「FLINT Middle School Group & Sunday Service」或「LIFELINE High School Group & Sunday Service」。

參加主日崇拜而不參加小組的青少年，請選擇「Sunday Service Only」。

您提交的表格在您的孩子從高中/Lifeline畢業之前有效。
